QUARTERLY REPORTS
Are you required to file quarterly reports with Department of Transportation?
If you answered, yes, to one or more of these statements below then you are required to submit reports to the appropriate agencies.
- Yes, I carry an IFTA decal sticker
- Yes, I carry the New York Highway Usage Tax Permit
- Yes, I carry the New Mexico Weight-Distance Number
- Yes, I carry the Kentucky KYU Number
- Yes, I carry the Oregon MCTD Permit *
* Permits that require a monthly reporting submission opposed to quarterly.
Reporting Timeframe
Quarter 1: January 1 - March 31
Quarter 2: April 1 - June 30
Quarter 3: July 1 - September 30
Quarter 4: October - December 31
Deadlines
Quarter 1: April 30
Quarter 2: July 31
Quarter 3: October 31
Quarter 4: January 31

Fleet - Mileage/Fuel Record Sheet
This form is best suited for fleet mananagement (companies that manage 2 or more power units) or individual carriers that prefer to track mileage on a per trip basis. It still allows carrier the ability to utilize the odometer reading method to track mileage and fuel recordings.
